WHY CREATINE MATTERS MORE DURING WEIGHT LOSS

Creatine's primary function is replenishing ATP (adenosine triphosphate) in muscle cells — the energy currency your muscles use during short, intense efforts like lifting. During a caloric deficit, ATP regeneration is compromised because your body has less available energy. Supplementing creatine partially compensates for this deficit, allowing you to maintain training intensity even as caloric intake drops.

But the lean mass preservation angle is what makes creatine particularly relevant during GLP-1 treatment:

Intracellular water retention. Creatine pulls water into muscle cells, increasing cell volume. This cell swelling is itself an anabolic signal — it tells the muscle fiber that it is being used and should not be broken down for amino acids. During a catabolic state (which GLP-1-assisted weight loss creates), this anti-catabolic signal is valuable.

Supports muscle protein synthesis. Emerging research suggests creatine may directly support muscle protein synthesis pathways (mTOR signaling) independent of its ATP-replenishing function. This effect is modest but meaningful when combined with resistance training during caloric deficit.

The scale context. Creatine causes 2 to 5 pounds of water weight gain (stored in muscle cells, not subcutaneously). This can confuse men who are tracking weight loss on a GLP-1. The water weight is functionally positive — it is inside muscle cells where you want it — but it can mask fat loss on the scale. If you start creatine while on a GLP-1, expect the scale to stall or bump up for 1 to 2 weeks before fat loss resumes. Use waist circumference, not just scale weight, to track progress.

CREATINE AND TESTOSTERONE

A frequently cited 2009 study found that creatine supplementation increased DHT (dihydrotestosterone) levels by 56% in college-aged rugby players. This single study generated years of both excitement ("creatine boosts testosterone!") and concern ("creatine causes hair loss through DHT!"). The reality:

The DHT finding has not been replicated. Multiple subsequent studies found no significant effect of creatine on testosterone or DHT. The original study had a small sample size and methodological limitations. Current consensus among endocrinologists is that creatine does not meaningfully alter androgen levels.

The hair loss concern is theoretical. Even if creatine did raise DHT (which subsequent data does not support), the relationship between DHT and hair loss depends on genetic sensitivity of hair follicles. Men without androgenetic alopecia genes would not be affected regardless. Men with active pattern hair loss may want to discuss with a dermatologist, but the evidence basis for creatine-induced hair loss is extremely weak.

For men on GLP-1s specifically, the testosterone picture is dominated by weight loss effects (increased T from reduced aromatase) — any theoretical creatine-DHT interaction would be dwarfed by the hormonal changes from losing 30 or more pounds.

PRACTICAL CONSIDERATIONS ON GLP-1s

GI tolerance. Creatine monohydrate can cause mild GI discomfort in some people. On a GLP-1, where GI sensitivity is already elevated, start with 3 grams per day for the first week and increase to 5 grams. Take it with food rather than on an empty stomach. Micronized creatine monohydrate dissolves better and may cause less stomach irritation than standard powder.

Hydration. Creatine increases intracellular water demand. On a GLP-1, where reduced food intake means less dietary water (food contributes roughly 20% of daily water intake), hydration becomes more important. Add 16 to 24 oz of water per day beyond your normal intake when supplementing creatine.

Kidney function. Creatine is safe for healthy kidneys — this is settled science supported by decades of research. However, creatine supplementation increases serum creatinine (a metabolic byproduct measured in kidney function tests). If your GLP-1 provider orders metabolic panels, flag your creatine use so elevated creatinine is not misinterpreted as kidney dysfunction.
