You started checking your Apple Watch or Fitbit more carefully after beginning semaglutide, and you noticed something unexpected: your resting heart rate went up. Not dramatically — maybe 3 to 8 beats per minute — but enough to trigger the anxiety spiral. Here is what the clinical data says about why it happens, when it matters, and when it does not.
THE CLINICAL FINDING
GLP-1 receptor agonists — both semaglutide and tirzepatide — consistently produce a small increase in resting heart rate in clinical trials. The STEP trials documented an average increase of 1 to 4 beats per minute for semaglutide. The SURMOUNT trials showed a similar 1 to 4 bpm increase for tirzepatide. Some individual patients experience larger increases of 5 to 10 bpm, particularly during the dose titration phase.
The mechanism is not fully established, but the leading hypothesis involves the sinoatrial node. GLP-1 receptors are expressed in the heart, and receptor activation appears to have a mild chronotropic effect — directly increasing the firing rate of the heart's pacemaker cells. This is independent of the sympathetic nervous system (it is not an adrenaline response) and independent of weight loss.
For most men, a resting heart rate increase from 65 to 70 bpm, or from 72 to 78 bpm, is clinically insignificant. The heart is designed to handle a wide range of rates — athletes operate at 45 to 55 bpm, sedentary adults at 70 to 85 bpm. A medication-induced bump of 3 to 8 bpm stays well within physiological norms for the overwhelming majority of patients.

A resting heart rate that stays above 100 bpm (tachycardia) warrants medical evaluation regardless of cause. If your GLP-1 pushes your RHR from 90 to 100+, or if you experience palpitations, chest discomfort, or shortness of breath at rest, contact your provider. These are not normal GLP-1 effects.
WHY MEN NOTICE IT MORE
Men are more likely to notice the heart rate increase for two reasons. First, men are more likely to wear fitness trackers that surface resting heart rate data prominently — and the transition from a consistent baseline to a slightly elevated reading triggers concern. Second, men who exercise regularly tend to have lower baseline RHR (55 to 65 bpm), making a 5 to 8 bpm increase proportionally more noticeable than the same increase on a baseline of 75 to 80.
The irony: men with lower resting heart rates (indicating better cardiovascular fitness) are more likely to notice and worry about the increase, despite being the population least at risk from it. A fit man whose RHR goes from 58 to 64 is still well below the average for his age group.

The pattern for most men: heart rate bumps up during titration, peaks when you reach your target dose, and then gradually comes back down as cardiovascular fitness improves from weight loss and increased activity. Men who combine GLP-1 treatment with regular cardio often see their RHR return to pre-medication baseline or below by month 6 to 12.
WHAT TO DO
Track, but don't obsess. Check resting heart rate weekly (not daily — daily variation is normal and creates noise). Record it in the same conditions: morning, seated, before coffee. A weekly trend is informative. A single elevated reading is not.
Don't stop your medication over 5 bpm. An increase from 68 to 73 is not a medical emergency. It is a known, expected pharmacological effect. Stopping a medication that is producing meaningful weight loss because of a clinically insignificant heart rate bump is a poor risk-benefit trade.
Mention it at your next check-in. Your GLP-1 provider should know about any heart rate changes, especially if the increase is greater than 10 bpm or if you have pre-existing cardiac conditions. They may check a basic ECG or adjust your dose timing.
Cardio helps. Regular aerobic exercise (30 minutes of Zone 2 cardio, 3 to 4 times per week) is the most effective way to lower resting heart rate over time. Combined with GLP-1-mediated weight loss, cardio can more than offset the medication's chronotropic effect.
