Two drugs. Two mechanisms. Two different results profiles. If you're a man deciding between semaglutide and tirzepatide, here's the data that actually matters for your decision.
Efficacy: Tirzepatide Wins on Weight Loss
The numbers are clear from Phase 3 data:
- Semaglutide 2.4mg (STEP-1): ~16.9% average weight loss at 68 weeks
- Tirzepatide 15mg (SURMOUNT-1): ~22.5% average weight loss at 72 weeks
A real-world JAMA Internal Medicine study confirmed the trial data: tirzepatide produced greater weight loss than semaglutide in head-to-head comparison in clinical practice, not just controlled trials.
Side Effects: Similar but Not Identical
Both cause GI side effects (nausea, diarrhea, constipation). Tirzepatide may cause slightly more nausea during titration but may be better tolerated at maintenance. Semaglutide has a longer track record and more safety data (including cardiovascular and kidney outcome trials).
Testosterone Impact
Both improve testosterone through weight loss. The ENDO 2025 data showing testosterone normalization (53→77%) was observed across GLP-1 classes generally. Greater weight loss with tirzepatide may produce faster testosterone normalization in men with obesity-related hypogonadism.
Cardiovascular Evidence
Semaglutide has SELECT trial data showing 20% MACE reduction. Tirzepatide's cardiovascular outcome trial is still pending. If you have established cardiovascular disease, semaglutide has the stronger evidence base today.
