Veterans and GLP-1s Beyond the VA: The 2026 Access Guide

The VA formulary for GLP-1s has specific eligibility criteria that many veterans meet — and many don't. Veterans who don't qualify or who can't accept VA wait times have several outside-VA pathways worth understanding. Here's the 2026 landscape.

VA Formulary Access

The VA covers semaglutide and tirzepatide for veterans meeting specific criteria — typically type 2 diabetes with A1c targets not met on first-line therapy, or obesity with defined BMI and comorbidity thresholds. Wegovy and Zepbound (weight-loss indications) have historically been more restricted than Ozempic and Mounjaro (diabetes indications).

Eligible veterans access these medications at low or no copay through VA pharmacy. This is the best-cost path if you qualify. Talk to your VA primary care team to determine eligibility.

VA Community Care

Veterans who face long wait times at their VA facility can, in some situations, receive care in the community with VA-funded coverage. This is called community care. For GLP-1s, community care coverage generally follows the same formulary rules as direct VA — you'll get the medication if you meet clinical criteria, at community pharmacy pricing negotiated by the VA.
