Why the Combination Works

GLP-1s drive weight loss but can accelerate lean mass loss along with fat mass if protein intake and resistance training aren't optimized. TRT, in the setting of adequate protein and training, protects and can slightly build lean mass. Dose Interactions

GLP-1s don't pharmacokinetically interact with testosterone, so there's no dose-adjustment needed for the medications themselves. But as weight comes off, some men need less TRT to hit target labs — visceral fat aromatase activity drops, which reduces the estradiol conversion competing with testosterone effect. Your TRT prescriber should monitor total T, free T, estradiol, and hematocrit at intervals and consider dose reductions as weight loss progresses.

Who Shouldn't Stack

Men with active prostate cancer, elevated PSA with unresolved workup, uncontrolled sleep apnea, or hematocrit issues should not add TRT to a GLP-1 without their oncology or urology teams involved. The combination isn't inherently problematic in these populations, but the TRT piece specifically requires careful evaluation.
