Who Should Choose GLP-1 Over TRT

GLP-1 medications are the better first-line choice for men with BMI ≥30 and testosterone below 300 ng/dL (especially if LH/FSH are low-normal, suggesting the brain is responding to estrogen from fat tissue), men who want to preserve fertility, men who prefer treating root causes over symptoms, and men who want to avoid lifelong TRT dependence.

TRT remains the better choice for men with primary hypogonadism (testicular injury, Klinefelter syndrome, etc.), men with normal BMI and genuinely low testosterone, and men who have already tried weight loss without testosterone recovery.
