The ~2% difference in efficacy versus injectable semaglutide is real but modest. For many men, the convenience and cost savings will outweigh a slightly smaller effect size.
WHY ORAL IS HARDER
Semaglutide is a peptide — a string of amino acids. Peptides don't survive the digestive system well; stomach acid breaks them apart before they can be absorbed. This is why most GLP-1s require injection.
Oral semaglutide works by combining the drug with SNAC (sodium N-[8-(2-hydroxybenzoyl)amino] caprylate), an absorption enhancer that protects the peptide and helps it cross the stomach lining. It's clever chemistry, but it comes with restrictions:
- Empty stomach required: Take it first thing in the morning with no more than 4 oz of water
- Wait 30 minutes: No food, drink, or other medications for at least 30 minutes after taking
- Lower bioavailability: Less than 1% of oral semaglutide is absorbed (vs 89% for injectable), which is why the dose is 25mg instead of 2.4mg
⚠️ Taking It Correctly Matters
If you don't follow the dosing instructions (empty stomach, minimal water, 30-minute wait), absorption drops significantly and the medication won't work as well. Compliance with the regimen is more important with oral than injectable.

The SOUL trial, published alongside the weight loss approval, showed oral semaglutide delivers cardiovascular protection similar to injectable:
